Registering with the surgery

To register with the surgery:

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

As part of the National GP contract, all patients now have a ‘named’ GP.

The patients at Dawes Family Practice are registered with either Dr Darling or Dr Majumdar.

You probably think that you already have a named GP or a GP whom you would normally visit at the surgery. This WILL NOT affect your choice of doctor.

If you see a particular doctor at the surgery, you should continue to see them.  You can also express a preference about who you want your named GP to be.

The ‘named’ GP will, however: –

  • Take lead responsibility for ensuring that all appropriate services required under the contract with the practice are delivered to you.
  • Where required, based on the professional judgement of the ‘named’ GP, work with relevant associated health and social care professionals to deliver a multidisciplinary care package that meets your needs.
  • Ensure that your physical and psychological needs are recognised and responded to by the relevant clinicians in the practice.
  • Ensure that you have access to a health check if requested, which is already a requirement of the GP contract regulations

Prior to travelling please allow as much time as possible to arrange your appointment for the Travel Clinic (if required and preferably at least 6 weeks or more), which will be with the Practice Nurse. The Nurse will require to know which countries, and areas within countries, that you are visiting to determine what vaccinations are required.

It is important to ensure you complete the Travel Risk Assessment form and submit this to the Practice at your earliest convenience. The Practice Nurse will then look to see what vaccinations you require and make contact with you to either advise you are all up to date, or make you an appointment at the surgery to have the vaccinations you require. If you are submitting these within 6 weeks of your travel, you may need to speak to the Travel Clinic for further advice and vaccinations as we may not be able to offer you a nurse appointment.
